vRA7.6 Customized User Interaction Forms

Hi

         i am building a XAAS blueprint.  There are user interactions in my workflow. I am running vRA7.6 .    in my vRO web client, i am able to customize the user interaction form like below.

     after initial the request, user is still seeing an not-well-formatted form like below.

           Did i miss anything?   any comments would be highly appreciated.
